Poland restored their two-goal cushion in the 76th minute when substitute Martyna Wiankowska curled a strike off the bar and in.
Denmark responded late via Signe Bruun in the 83rd minute, heading home from a well-delivered corner to set up a tense finale.
Poland held firm in the final minutes to seal a historic win and bring their exit to a triumphant close.
